REQUEST FOR PROPOSALS
Notice is hereby given that sealed proposals will be received by the County of Hudson, State of New Jersey, 567 Pavonia Avenue -3rd Floor of the Board of the Chosen Freeholders Jersey City, NJ on Tuesday, February 26, 2013 11:00 a.m. prevailing time, and at that time and place the following will be publicly opened and read:
Area Plan 2013-2015 Services to the Elderly Hudson County Area Agency on Aging Contract Period: April 1, 2013 to December 31, 2015
With up to two (2) one-year extensions
1. Access Services:
Information & Assistance, Benefits Screening, Extended Assessment, Outreach, Care Management, Transportation, Assisted Transportation, Language Translation & Interpretation
2. Home Support Services:
Friendly Visiting, Telephone Reassurance, Residential Maintenance, Housekeeping, Certified Home Health Aide, Hospice, Emergency, Respite Care
3. Community Support:
Adult Day Care – Medical, Legal Assistance, Physical Health, Oral Health, Mental Health, Physical Activity, Education, Socialization/Recreation, Caregiver Support, Caregiver Mental Health Counseling
4. Nutrition:
Ethnic Meals – Congregate (including Kosher), Ethnic Meals – Home Delivered (including Kosher)
A bidder’s conference will be held Friday February 8, 2013 at 11:00 a.m. at the Hudson County Meadowview Campus Conference Room, 595 County Avenue, Building #3, Secaucus, NJ 07094, basement level.
Requests for proposals may be obtained through the Hudson County Office on Aging website, http://www.hudsoncountynj.org/request-for-proposals-rfp-information.aspx. Hard copies of the RFP will not be available at the conference. It is suggested that the applicant download a copy of the RFP and bring it to the conference. Proposals shall be submitted on the forms provided and shall be in accordance with the Request for Proposals.
Proposals are being sought for services as outlined above for senior residents in Hudson County, NJ. Funding sources for these services include Older Americans Act (Title III), Social Services Block Grant (SSBG), other State funds, and/or County funds.
Proposals and one (1) CD/DVD or USB Drive which contains a secure copy of the complete proposal in PDF, WordPerfect, or Microsoft Word format must be hand-delivered at the designated time for receipt of proposals. No proposals will be accepted prior to the designated time or at other locations. All proposals must be contained in a sealed envelope or box. Required language to be written in ink on the exterior of all envelopes or boxes is as described on page 17 in the General Conditions and Instructions to Applicants. No late or faxed bids will be accepted.
Applicants are required to comply with the requirements of PL 1975, c. 127 (NJAC 17:27), Affirmative Action Plan, Ordinance 559-12-2011 and PL 1977, c. 33, Disclosure Statement, and all other requirements set forth in the General Conditions and Instructions to Applicants.
Applicants are required to use the statement forms contained in the General Conditions and Instructions to Applicants.
This proposal is being solicited through a “Fair and Open” procurement in accordance with N.J.S.A. 19:44A-20.4 et. seq.
The County of Hudson reserves the right to reject any and all proposals, to waive any informalities, and to accept a proposal, which, in its judgment, best serves the interest of the County. No proposals may be withdrawn for a period of sixty (60) days after the date and time set for the opening of the proposals.
